A head-to-toe exam, and completion of physical examination forms.

Blood pressure, cholesterol, diabetes, and cancer screenings as recommended.
Chronic condition care
Regular follow-up, medication management, and lab monitoring.
Vaccines
Recommended immunizations, guided by your age and health history.
Referrals & coordination
Connections to specialists, imaging, and labs — and follow-up afterward.

Bring your information
Your insurance card, photo ID, a list of medications, and any forms you need completed.

See your physician
We listen, examine you, and agree on a plan — including any tests, prescriptions, or follow-up.
Vaccines
Recommended vaccines for adults.
Our team can help you decide which vaccines are right for your age, health history, and lifestyle. Availability of specific vaccines varies — please call ahead to confirm.
Influenza (flu)
Recommended every year for most adults.
COVID-19
Updated vaccines as recommended for your age and health.
Pneumococcal (pneumonia)
For adults 50 and older and younger adults with certain conditions.
Shingles
A two-dose series for adults 50 and older.
Tdap / Td
Tetanus, diphtheria, and whooping cough protection, with boosters every 10 years.
Hepatitis B
For adults who have not been vaccinated, especially those at higher risk.
Medicare Part B covers flu, COVID-19, pneumococcal, and hepatitis B vaccines for eligible people; Medicare Part D covers other recommended adult vaccines, such as shingles and Tdap, generally with no cost-sharing. Most commercial plans and Medicaid cover recommended vaccines as preventive care.

Insurance & coverage
What your plan may cover.
We accept most insurance plans, including Medicare and Medicaid, and nobody is turned down. Preventive visits such as the Medicare Annual Wellness Visit and many screenings are often covered with no cost-sharing; sick visits and follow-ups may involve a copay or coinsurance.
Note that Medicare covers an Annual Wellness Visit rather than a full "annual physical" — ask us if you are unsure which visit you need.

Do you accept walk-ins?
Yes. Walk-in and urgent care visits are available from 5 PM to 9 PM every day. Scheduled visits run from 9 AM to 5 PM.
Can you complete my physical examination form?
Yes. Bring the form to your physical, or complete our online Physical Examination Report before your visit.
Which vaccines do you offer?
We offer recommended adult vaccines such as flu, COVID-19, pneumonia, shingles, Tdap, and hepatitis B. Availability varies, so call ahead.
What is the difference between a physical and a Medicare Annual Wellness Visit?
A Medicare Annual Wellness Visit focuses on prevention planning and screenings; a physical includes a full hands-on exam. We will help you schedule the right one.
Are you open on weekends?
Yes. Our practice is open seven days a week, including Saturdays and Sundays.
Do you offer telemedicine?
Yes, for many concerns, during extended hours from 5 PM to 9 PM.
